Recreational Supplemental Field Training Request Form

Please complete this form if your club is interested in hosting a recreational field training to help your first time coaches receive a practical in person training. Once the application is complete our Cal South staff will follow up to set up an event date at your club field.
Please read and fill in the information in its entirety to ensure proper registration for your coaching event.

Requirements to host a coaching event:
- This form must be submitted at least 30 days prior to your event date
- Minimum of 15 participants
- Classroom, field, lights (if applicable), bathrooms, first aid kit
- Host at event to provide permit if needed
- Host must provide one (1) team.
