Parent(s) of kids under 5 years old must remain within the facility during the session.
To waive all claims that I have or may have in the future against B-Ballerz Athletic Academy, its members, employees, volunteers, and independent contractors (all of whom are hereinafter collectively referred to as “B-Ballerz Athletic Academy”).
To release B-Ballerz Athletic Academy from any and all liability for any loss, damage, injury, or expense that I may suffer, or that my next of kin may suffer as a result of my participation in the activity of basketball skills training, due to any cause whatsoever including negligence, breach of contract, or breach of a statutory or other duty of care.
Please ensure all children's belongings are clearly labelled with their name.
To hold harmless and indemnify B-Ballerz Athletic Academy from any and all liability for any damage to the property of, or personal injury to, any third party resulting from my participation in a basketball training session.
This agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, assigns, and representatives in the event of my death or incapacity.
I, the undersigned, hereby authorize B-Ballerz Athletic Academy to photograph, take motion pictures of, take video footage, and/or make electronic sound recordings. I understand that these pictures and/or videos may be posted on B-Ballerz Athletic Academy’s social media accounts including Facebook, Instagram, and other accounts.
In entering into this Agreement, I am not relying upon any other oral or written representations or statements made by B-Ballerz Athletic Academy other than what is set forth in this Agreement.
I have read and understood this agreement and I am aware that by signing this agreement I am waiving certain legal rights which I, or my heirs, next of kin, executors, administrators, and assigns may have against B-Ballerz Athletic Academy.

NO OUTDOOR SHOES ALLOWED IN THE ACADEMY. All outdoor shoes must be removed. I understand that parents and kids can only enter the academy with appropriate Indoor Athletic/Training shoes.
