Once you've filled out this form, please submit payment through either Venmo or Chase Quickpay. PAYMENT IS DUE WITHIN 1 HOUR, AS ITEMS ARE FIRST COME, FIRST SERVE.

Cookie pickups will be made from my house in Deerfield. I will send you the address when I confirm receipt of your payment.
