Effective March 15, 2023 all clients must abide by the Client Policies & Operative Proceedures. When scheduling an appointment you acknowledge that you agree to the following terms:
Medical History: It is the client's responsibility to disclose the following and any additional information that may be pertinent to the horses wellbeing during a PEMF session.
- If the horse has been recently diagnosed with EPM or Toxicity.
- Any new medications being adminstered.
- Any injections (intramuscular or intraveinous) that have been given less than 24 hours of session.
- Any vaccines given in 24 hours of session.
- All Intra-articular (joint) therapies given in the last 3 days
- Pregnancy
- Cancer
- Active infections
- Any active bleeding internally or externally.
- Blood disorders
- Blood clots, stroke or blood pressure disorders.
- Seizures
- De-nerved limbs
I understand that it is my responsibility as the owner, lesee, or agent of this horse(s) to notify my practitioner of any changes that have taken place.
By signing this agreement I am aware that complications can occur despite the best safety practices
Cancellations and/or missed appointments:
Please allow for 48 hours advanced notice for changes in scheduling. There will be every effort made to help reschedule or change your appointment, but anything less than 24 hours notice will incur a $50 charge. Please note, you can put another horse in your horses place (that resides at the same barn) to avoid this charge.
Payment:
- The client assumes full financial responsibililty for all charges agreed upon and incured during their session and payment is required at the time of service.
- Payment can be made via cash, check, venmo, zelle, or debit/credit card via invoicing.
- Prepaid sessions must be used within 4 months of purchase.
- Bounced checks will incur all fees associated with them.
