BC Games Incident Report Form

Use this form to report a situation where you have felt uncomfortable or there is a violation of the BC Games Society Culture of the Games or Code of Conduct.

Note
Generally a report should include the name and contact information of the person filing the report and a brief description of the occurrence, including the date, time, and location. An individual submitting a report may include any relevant evidence, including the names of witnesses.

The BC Games Society will try to obtain an informal voluntary resolution to all incidents at the lowest level possible.

The reporter's identity shall be kept confidential except to the extent necessary to conduct an investigation and/or carry out the BC Games Discipline Process.

Anonymous reports may be made but note that action on incidents may only possible with details, including who is making the report.
