    Medical Training Initiative
  • This form provides evidence that the MTI doctor has taken up the post for which they have been provided a Temporary Work - Government Authorised Exchange visa. It must only be completed by the doctor's employer, NOT the doctor themselves. Any starter report submitted by an MTI doctor will be rejected by the Academy.

      • Added clarification in line with UKVI's eVisas - As of 15/07/2025, UKVI will no longer provide physical visas for the Temporary Work - Government Authorised Exchange visa route, all visas will be eVisas.
      • Added this information to the question regarding whether the doctor applied from certain nations. This question will remove the visa vignette section from the report.

      • In certain circumstances, a migrant may not have a physical TW GAE visa page in their Passport, and will only have an eVisa instead. In order to account for this possibility, we have added a new section to this report to reduce confusion regarding the "entry visa vignette" date.

        • If the doctor's visa is issued for 6 months or less, they will be unable to create a UKVI account and therefor inelligible for a Right to Work profile. In these cases, an ECS check must be performed and submitted with the Starter report

    • Part 1: Details of the MTI doctor and Supporting Documents

      Please give all names EXACTLY as they appear in the passport
    • Please ensure the MTI doctor's name is included EXACTLY as shown on the Passport as the information contained in this report is submitted to UKVI.

       


    • If the doctor's NHS email address is not yet available, please provide their personal email address.

      In these cases, we will "request more information" once we receive the report, noting that the report should be resubmitted with the doctor's NHS email address once it becomes available. 

       

      Please note that we will not consider a Starter report to be complete until it contains the doctor's NHS email address.

    • We can only accept a Data Protection Consent form that was generated using the form on our website here: https://mti.aomrc.org.uk/mti-forms/

       

      1. The MTI doctor must complete the AoMRC Data Protection Consent form, including their UK address

      2. When the form is submitted using the green "Submit" button, the MTI doctor will be emailed a PDF version of the Data Protection Consent form to the email address they included on the form

      3. The MTI doctor must then forward this document to the UK employer who will attach it to this form

      4. The AoMRC will not accept any Data Protection Consent form that was not completed on the AoMRC system. 

       

      The Data Protection Consent form must be completed by the MTI doctor themselves and we cannot accept a form completed on their behalf. 

      Due to this, we cannot accept a screenshot of the form or an edited version of the document, we can only accept the PDF generated by our secure online form.

       

      We also cannot accept the Data Protection Consent form that was used for the doctor's original MTI application.

      As of 15/07/2025, all Temporary Work - Government Authorised Exchange visa holders will no longer receive a physical visa and this has been replaced by the new eVisa system. 

      Once the doctor receives their visa decision, they should log in to their UKVI account to check their eVisa status and once they have entered the UK, we will need a copy of the eVisa screen which should include details such as the start date of the doctor's visa.

      As the doctor's visa is for 6 months or less, we will require an ECS (Employer Checking Service) for this doctor, in order to establish the doctor's right to work in the UK.

      Once you have the document, please add your signature to the document to verify this as a true likeness of the doctor (we can accept a digital or physical signature, but it must be visible on the document itself, we cannot accept verification by email or an accompanying document). 

       

      Please note: We can only accept an ECS check in cases where you are unable to generate a Right to Work (RTW) profile. If we believe that a doctor should be elligible for a RTW profile but an ECS is provided instead, we will reject this report.

  • Please Note: The MTI doctor must commence their placement within 28 days of whichever is the latest of the following, otherwise they are considered late:

    • the latest start date displayed on their CoS (including Sponsor Notes)
    • the date the worker is notified of a grant of entry clearance or permission to stay
