Connext Care LLC may have to exchange PHI when implementing the various services. To provide these services, we and   First Name   Last Name   agree to abide by the following principles to maintain confidentiality and to follow HIPAA laws.

All individually identifiable information in any format ( written, spoken, faxed, or emailed) is considered PHI and and therefore will be utilized or transmitted following the list of safeguards outlined in DCF, and HIPAA training on the use and disclosure of PHI.


Connext Care LLC and   First Name   Last Name agree to share only minimum information necessary for the purpose of treatment, payment, and operation.


Connext Care, LLC and   First Name   Last Name agree to adhere to the principles of protection of consumer information and consumer rights, as outlined in the notice of Privacy Practices.
