KENTUCKY CHANGERS 2023 WORK SITE TRANSPORTATION

Transportation will be provided by utilizing any church vans and personal vehicles of adult participants. Each church is required to provide enough transportation to carry the number of students and adults you bring to Changers. We may use 12 or 15 passenger vans, mini-vans, SUV’s, trucks, mini-buses, or cars.

Each Adult (21 years or older) that is approved to drive one of your group’s vehicles will need to be listed on the Approved Driver’s List. Each person on this list will receive an email directing them to the Protect My Ministry website that Kentucky WMU is using to conduct Motor Vehicle Record (MVR) checks on all drivers. At this web page, drivers will input the information needed for the MVR to be completed.

We also need information about the vehicles you will be using for transportation. This information will be submitted via the Transportation Survey Form.

Please send to KY WMU by June 1. An Approved Drivers List must accompany this form.

This address will be used to send each driver a link where they will fill out the consent form for their motor vehicle record (MVR) check.
