TERMS AND CONDITIONS
Please read the following terms and conditions carefully. These terms and conditions apply to your booking with Alyce Merrilees ABN#31196377914 (The Mindset Adventure, we us). By filling in this form you are irrevocably agreeing to enter into a contract with us on these terms and conditions. This contract becomes binding immediately on our confirmation of this form being completed.
DEFINITIONS
“You” refers to the guest booking a retreat. “We” and “us” refers to Alyce Merrilees facilitating The Mindset Adventure.
HEALTH AND SAFETY
You must declare any health or medical conditions to us at the time of your booking and where necessary update us any time if your circumstances change. Based on these disclosures we may require you to provide a medical certificate confirming that it is safe for you to participate in specific events. Despite these disclosures and any medical certificates provided, you are wholly responsible for your own health, safety and wellbeing at all times during the events, including during all activities. You are free to stop or decline participating in any activity at any time for any reason.
As with all physical activities, participating involves some degree of physical and mental risk. Know your limitations, both physical and mental, and act accordingly. If you have any concerns contact us.
Do not participate in any of The Mindset Adventure events if you have heart conditions of any kind, or are pregnant. Contact us first before booking to discuss options for you.
If you choose to continue or share in any way any of the knowledge or activities you learnt or perceive to have learnt from The Mindset Adventure you do so at your own risk and claim full responsibility; You agree to not hold The Mindset Adventure or any of its facilitators, representatives etc responsible for injuries, damages or death you may incur as a result.
Be aware of your surroundings if practicing any activities from The Mindset Adventure. Never practice in a place where losing consciousness would be dangerous (such as in water or driving a vehicle).
We are not doctors. None of this is medical advice, nor is it intended to diagnose, cure or treat any medical conditions. Any suggestions or information we offer is purely educational, and should not be regarded as medical expertise. For qualified medical advice please consult a medical professional.
PHOTOGRAPHY
During our events we do take photos and videos that may be used in future content or promotional material. We are mindful of your space, and will not take any footage at inappropriate times. If you do not want to be in any photos etc please let us know so that we can remove you from any content in the future.
PERSONAL LIABILITY STATEMENT
“I hereby declare that I have completed the health and declaration and that I participate in the workshop voluntarily and entirely at my own risk. I shall not hold The Mindset Adventure liable for any damages/injury/death resulting from participation in any of The Mindset Adventure workshops”
HEALTH DECLARATION
“I hereby declare that I have been adequately informed about the particulars of the The Mindset Adventure workshops beforehand and that I am in good mental/physical condition. I shall not hold The Mindset Adventure liable for any damages/injury/death resulting from participation in any of The Mindset Advenutre workshops/events. My participation is entirely at my own risk”
