Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by K WHITE TIGERS during the selected camp. In exchange for the acceptance of said child’s candidacy by K WHITE TIGERS ., I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless K WHITE TIGERS . and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against K WHITE TIGERS . including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including basketball. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

Program Consent and Authorization

Agreements and Permissions:
By signing the registration form, you agree to all policies and procedures of the K WHITE TIGERS MARTIAL ARTS program. Your child also agrees to abide by the K WHITE TIGERS MARTIAL ARTS Rules and Regulations.

Your signature allows K WHITE TIGERS MARTIAL ARTS to transport your child to and from school. Understand that your child’s name, image, and/or quotes may appear in promotional materials.

Medical Situations:
In case of a medical emergency, efforts will be made to contact you or your emergency contact. If unreachable, K WHITE TIGERS MARTIAL ARTS has permission to secure necessary medical treatment for your child, including hospitalization.

Risk Acknowledgment:
You assume the risk for any injuries or illnesses resulting from your child’s participation in activities. K WHITE TIGERS MARTIAL ARTS is not responsible for personal property lost or stolen on their premises.

Tuition Payment:
Tuition is charged monthly (flat rate for 4 weeks) regardless of attendance.
The first month’s tuition or prorated amount is due at registration.
Regular monthly tuition will be automatically deducted from your checking account or credit card.
Cancellation requires one month’s written notice, and refunds are prorated based on individual pricing.
