AUTHORIZATION AGREEMENT FOR ELECTRONIC FUNDS TRANSFER (EFT)
I (We) hereby authorize Blue Ridge Christian School to initiate debit entries to my (our) bank account indicated and the financial institution named below, to debit the same such account. I understand that this debit will include the cost of my tuition and book fees. If you would like for additional fees (aftercare, lunch, athletics, etc.) to be debited, please indicated by answering yes and initialing below.

If you had EFT for the 23-24 school year and your account information is the same, I do not need a voided check. If you are new to EFT, we must have your signed authorization form and voided check completed by the 15th of the month prior to the month in which you wish to begin having funds debited.

This authorization form is in effect until June 30, 2025 or until Blue Ridge Christian School receives a notification from me of its termination in such time and in such manner to allow BRCS a reasonable opportunity to act on it.
