Members-in-Training should not use this form to order a WHITE replacement badge or an initial GREEN badge. instead, contact the Initial Training Director for assistance.

Members-in-Training will automatically receive a GREEN badge upon completion of initial certification requirements. If initial certification occurs after class graduation, the green badge will be mailed in the month after initial certification is earned.
