  • Submit this form to notify the court that you intend to appear remotely via Zoom for your Traffic arraignment.

    This form is required to be submitted no later that the Friday before your appearance date. Submission of this form will indicate to the court your intention to appear remotely for your traffic arraignment - adding your case to the traffic court calendar for your appearance date. Failure to submit as directed will result in your case not being added to the calendar.

Until the citation has been processed by the Court, we will NOT have any information regarding your citation and will not be able to process your request.

    You are requested to submit this form no earlier than 30 days before your scheduled appearance date, unless you have already found your case information on the Online Case Index & Calendar Portal. You may return and submit your request after 30 days from the date of issuance; however, if your citation has not yet been entered into our system at that time, you may be asked to resubmit your request at a later date.

    Submissions of this form are not accepted if your citation was issued less than 14 days ago. Please return to this form after no less than 30 days have passed from the date your citation was issued to submit a Traffic Arraignment Calendar Add-On Request.
