Submission of this form will indicate to the court your intention to appear remotely for your traffic arraignment - adding your case to the traffic court calendar for your appearance date. Failure to submit as directed will result in your case not being added to the calendar.

You may return and submit your request after 30 days from the date of issuance; however, if your citation has not yet been entered into our system at that time, you may be asked to resubmit your request at a later date.
