A copy of the information in this form will be sent to OSCA for use by the coaches to nominate you. The 2024 year nomination meetings will be held April __, 2024 and this form must be submitted by April 22, 202_ in order to be eligible for nomination. Please PRINT your nomination form using the PRINT button on the final page of the form. After printing the form, check your form for missing information or incorrect spellings and errors. By submitting this form I agree to attend the appropriate All-State match to be held at Newcastle HS in Newcastle, OK as follows: 6A, Thursday June 8th, girls play at 6pm, boys at 8pm; 5A, Friday June 9th, girls play at 6pm, boys at 8pm; 3A, Saturday June 10th, girls play at 1 pm, boys at 3 pm; 4A, Saturday June 10th, girls play at 6pm, boys at 8pm. Failure to attend the game will remove you from the All-State roster. If you cannot attend the game, inform your High School coach ASAP as soon as your receive the letter announcing your selection so a replacement player can be selected.
