Please fill out form for each person that is coming from your house hold. Minor has to be filled out and sent seperate from parent if parent is attending. As you would like it to appear in Certificate and Wallet CPR Card
If you would like to E-Transfer please click on the Forms tab on the wesite and download registration.
Refund Policy: Course fees are non-refundable, but may be transferred to another course date with 30+ days notice or to another person for the same course with less than 30 days notice. By registering you are agreeing to our Refund Policy.
No refund will be given if cancellation is given with in the 30 days. But option to transfer to another up coming class.
No shows will forfeit payment and class will not be available for transfer to another date.
