This form is to document the assets that are assigned to you by LiNK Health Group for authorized business use only.
You are responsible for protecting LiNK’s assets within your home office environment and when travelling for business purposes. We recommend regular care and cleaning of equipment to ensure longevity. If your equipment becomes damaged, please contact the Helpdesk as soon as you become aware so we can provide a workaround or have your equipment collected to be fixed. Failure to do so may result in the reclaiming of costs from you directly. Spot checks will be carried out during the year to assess the condition of equipment.
LINK’s travel insurance policy covers assets outside of your home office environment when you are travelling for business purposes only. If you choose to take your equipment during personal travel, you will be liable for any damage or loss obtained.
Upon leaving the company, all Company property assigned to you as listed below must be returned to the Company on your last working day (this will be done by a courier as arranged by LiNK). By signing this form, you are agreeing that LiNK has the right to deduct the cost of any repair to this equipment should it be returned damaged, or the cost of a replacement should it not be returned at all. This cost will be deducted from your final salary, and you will be advised in advance of the exact cost prior to the deduction being made.
