Cancellation Request Form

I understands that membership dues are non-refundable, paid in advance of service, and that their membership will be automatically renewed at the end of each term unless a 30-day prior online membership cancellation form is submitted and the below membership cancellation process is completed.

Memberships of any term will not be refunded or transferred for any reason. Early termination of memberships (this applies to any membership option) prior to the expiration date are non-refundable.

To cancel any membership, the member is required to complete 3 steps. If the member fails to complete one or more of the 3 steps in the cancellation process, the membership will not be canceled and the member will continue to pay their membership dues.

The steps to cancel are as follows:

Submit a cancellation request via the CrossFit Mischief’s website and schedule phone exit interview

Attend the phone exit interview

Complete the online membership cancellation form

Once I has completed all 3 steps and the cancellation process is complete, this will serve as my 30-day written cancellation notice.

My membership will be canceled 30 days from the submission of the final form (membership cancellation form). If my membership dues fall within the 30 day notice, I will be responsible for this payment and that the membership dues will be automatically processed.

There is a $80.00 membership reactivation fee if a member wishes to reactivate their account.
