Mashantum Tennis Club - Junior Ladder Registration Form

2023 Season - June 27 through August 19
Schedule: All Ladders are held Monday through Friday (5 Days), except Ladder One has Junior Interclub Match Play on Fridays in place of Ladder. Private & Family Group lessons are offered as well. The Schedule is as follows:
- Ladder 4 (ages 5-8) -- 9:30 AM to 10:30 AM
- Ladders 2 and 3 (ages 9-12) -- 10:30 AM to 11:30 AM
- Ladder 1 (ages 13-17) -- 11:30 AM to 1:00 PM (Mon-Thurs); Fri 1:00 PM to 3:00 PM Interclub Match
Please register in Ladder based on age - each Ladder will be divdied into several groups each day, based on skill level - feel free to confer with Head Pro, however, adjustments can & will be made as needed
RATES (Full Non-Member Rate) - $145.00* per week

*LESS applicable Member Discounts calculated at the end of the form

*DISCOUNTS for Concurrent Memberships: 20% DISCOUNT for Famly Memberships; 10% DISCOUNT for other membership types

For value received, I/we agree, on behalf of myself/ourselves and my child/children named herein, and my/our heirs, successors, guests and assigns, that I/we assume all risks and waive any liability of any nature whatsoever against and agree to indemnify and hold harmless the MASHANTUM TENNIS CLUB, INC., its officers, directors, members, employees and agents (“CLUB”) with respect to any and all actions, claims or demands that may be made or brought on my/our behalf against said CLUB, its officers, directors, members, employees and agents in connection with my/our and/or my/our child’s/children’s participation at the CLUB while in the lesson program, playing or watching tennis, or any related activities thereto, or utilizing any of the land (including coming to or leaving from the CLUB property), buildings, equipment, furniture, fixtures and facilities of said CLUB, for personal injuries, property loss or damage or any other claim. THIS WAIVER AND RELEASE SHALL ALSO EXTEND TO THE DENNIS CONSERVATION TRUST AND ITS TRUSTEES. Furthermore, I/we agree to release and allow the CLUB to use any photographs and/or videos take of me/us, and/or of my/our children, while at the CLUB, for CLUB advertising purposes only, either online or in print form. Furthermore, I authorize my above listed email to be added to the Mashantum Tennis Club Email Contact List for Club emails. I/we have fully read and understand the Waiver and Release, & its terms and conditions, & freely and voluntarily agree that it is binding upon me/us, my/our child/children & our heirs, successors, guests and assigns. In case of medical emergency or general medical care, I give consent for medical treatment for the named junior participant(s) by authorized personnel. I certify that my child has permission to participate in all activites.
