This form is to be completely filled out and signed by a parent or legal guardian before any student of King's Academy may be transported by King's Academy / His Providence Church staff or volunteers. This form will serve as consent to transportation to/from field-trips and any off-site event including lunch and/or recess activities. Permission will remain active until revoked in writing by parent or legal guardian.

I,   First Name   Last Name   hereby give my permission to King's Academy to transport the children listed above to and/or from school events, including lunch and recess activities. I understand that my children will be under adult supervision. I further understand that in signing this form, I release and hold harmless King's Academy and His Providence Church, it's board, elders, officers, employees, interns, and any volunteers from any liability, past or future, fully and completely. I also authorize the staff or designated medical professionals and/or volunteers to administer emergency medical assistance if I cannot be reached.
