ACVR-EDI Annual Update Form for Residency Programs
Purpose: To provide current information to ACVR and the Residency Standards and Evaluation Committee (RSEC-EDI) on the status of each residency training program.

This form is to be completed by January 31 of each year. The form should be completed by the radiology residency program director. The program with be reviewed by RSEC-EDI and any questions and/or concerns will be directed to the program director
