Community Needs Survey

Welcome,

Goshen HELP DBA WYO HELP appreciates your participation in this survey as part of our work with the Community Service Block Grant (CSBG) program. The results of this survey will help us prioritize funding areas based on the greatest needs.

In addition to general questions, we ask you to identify the most important needs of your community based on specific categories. Please take the time to review each question carefully before making your decision. As always, we value your input and your feedback is confidential. The estimated time to complete the survey is approximately ten (10) minutes. 

You must answer every question. If the question doesn't apply to you please enter a zero or choose the Not Applicable option.

 

This survery is only for those living in Carbon, Crook, Goshen, Niobrara, Washakie, or Weston Counties.

Below are the Federal Poverty Level Guidelines for 125% and 200%. Household income includes employment, self-employment, Social Security, retirement, disability, worker's compensation, unemployment, VA benefits, and alimony. Please reference the table below for the questions that follow.
