CP2P Officials

THIS AGREEMENT, made and entered into on   Date   by and between   First Name   Last Name  ,      an Independent "At Will" Contractor, herein after referred to as "IAWC" and CORONA PYLON 2 PYLON, herein after referred to as "CP2P".  

WHEREAS, IAWC desires to work as a qualified flag football official and WHEREAS CP2P is qualified to offer quality games to officiate.

IAWC agrees & understands the CP2P terms and conditions as written below:
1. Accept all assigned games per the CP2P communication platform provided.
2. To not turn back assignments without a 24 hour notice. Failure to do so or just "Not Show", will result in a $20 fine for less than 24 hour notice & $35 for "No Show".
3. Work a tiered game fee system. Accept "base" rate for all games as discussed via verbal, text and/or email with CP2P.
4. Have the right to turn back assignments based on game fee offered or availability.
5. Arrive on field ready to work no later than 15 minutes prior to the start of my scheduled shift. This does not mean "I'm in the bathroom; in the parking lot; at the snack-bar; etc. Not complying will result in a $15 fine for 1st occurrence.
6. Be in the prescribed uniform i.e. CP2P Jersey, black shorts or pants and black shoes. Failure to do so will result in a $5 fine for 1st occurrence.
7. Be compensated within 24 hours of the finished scheduled shift.
8. Must have signed and submitted, this agreement.     
9. Must have submitted W9 form.
10. Must have submitted Work Rules form.      
11. Your $25 dues will be paid through your 1st game fee compensation.
Dues will include:

• CP2P Fox 40 whistle
• CP2P Officials' Jersey

12. All forfeits will be paid in full.
13. One person games will be paid at 1.5x your rate.
14. Be removed from any and all game assignments for any reason without cause as an independent "At Will" contractor.

IAWC agrees to indemnify, defend and hold harmless CP2P, its officials, representatives and volunteers from any and all claims, demands, defense costs, liability or consequential damages of any kind or nature arising out of connection with CP2P (or CP2P's subcontractors, if any) performance or failure to perform, under the terms of this contract; excepting liability actions arising out of sole negligence of CP2P.
