Participation/Medical Release Agreement
While we make every effort to provide a safe and pleasant environment for everyone in attendance, we understand that sometimes there are incidents that are beyond our control. We ask that you would please sign this waiver and also if you are 19 and under, please have your (parent/guardian sign on your behalf).
With full knowledge, I accept full responsibility for any injury or accident that may occur while participating in any PHBC Women's Ministry Spiritual Retreat activities that I voluntarily engage in or as I move throughout the building and campus from one room or location to another. Any and all areas while on PHBC's campus within or without the facility. (As I sign on behalf of my daughter/s 19 and under, I give her/them permission to participate in activities that occur at the PHBC Women's Spiritual Retreat.)
*All games, activities and crafts will be safe, non-strenuous and there is no foreseen harm or threat of danger for anyone.
Pleasant Hill Baptist Church Women's Ministry & it's Leaders have taken the necessary steps and precautions to provide games, activities and crafts that will promote Christian Fellowship, Fun and camaraderie for all Women and Youth with no risk involved. We encourage everyone to engage safely and seek the best interest of others. Although we may become excited while engaging in said activities, we ask that we are mindful of others around us, not allowing ourselves to create any loss or damage of property, accidental injury or even illness.
I, on behalf of myself, my child/ren, my assigns and my estate, agree to release and hold harmless Pleasant Hill Baptist Church, Pastor, Leaders, Women's Ministry Officers, Board, Agents or Employees, for any and all claims for injuries, causes of action, or liability related to my or my child/ren's participation in any activity occurring at Pleasant Hill Baptist Church.
This release does not apply to intentional and/or willful acts of misconduct by Pleasant Hill Baptist Church or any of the aforementioned parties.
Should Pleasant Hill Baptist Church, or anyone acting on their behalf, be required to incur attorney's fees and costs to enforce this agreement, I agree to indemnify and hold Pleasant Hill Baptist Church harmless for all such fees and cost.
By signing this document, I acknowledge that if anyone is hurt or property damaged during my or my child/ren's participation in these games, activities and crafts, I and/or my child may be found by court of law to have waived any right to maintain a lawsuit against Pleasant Hill Baptist Church on the basis of any claim which has been released herein.
I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound to its terms.
