This is a hidden field created solely to document the features of this form for the author and future maintainers. This field should stay hidden.
Form Purpose: Register playes for tennis and act as a template for future sports registration forms.
Before posting for a new season/sport, revisit the charges to discern if an an increase or decrease in costing is meritted.
The form is integrated with a google sheet which allows coaches to assign jersey numbers and document the return of jerseys.
Submissions are deleted after 6 months so there are no entries for the upcoming year.
Upon seeing that there is a submission for a sport, someone should have the job of adding the student and his parent to the google group for the sport. Adding the membership to the google group could be automated but that is not the case right now.
To Clone this form for other Sports:
1. In the My Drive for the dumb account, make a copy of google workbook "Sport Registration Form" and then rename the copy to " Registration Form" where is the capitalized name of the sport for which you are making a new registration form. (You must log into google drive with the dumb account to take this step and the next step)
2. Change the share settings for the new google workbook to 'Anyone with the link'
3. Clone this form, renaming it to the same name as the google sheet from step 1 above. Update the title of the form to match the same name as well.
4. Under settings-->integrations of the newly cloned form, click the 'google sheets' integration (If there is already a google sheets integration, click it, and then click 'Remove Integration'). When creating a new integration, it will ask which google account to use. Use the 'dumb' account. Then it will ask you which spreadsheet to connect to ... choose the new google workbook that you created in step 1. It will ask which sheet of the workbook to use ... enter "Form Responses". It will ask you which fields to transfer to the google sheet ... by default, all the fields are chosen ... don't remove any fields.
5. Click the done button to finish setting up the integration.
6. Fill out the form with faux information to test it (There is a coupon code that will allow you to fill out the form without paying the fee ... the coupon code changes with each clone of the form so you will have to navigate to that field in the form editor to determine the coupon code). When the new form is completed, there should be an entry in the 'Form Responses' sheet of the google workbook and there should be an entry in 'Sheet1' as well. Copy the link to sheet 1 and share it with the coach via athletics@chestertonannapolis.org so they can record jersey numbers assigned to the athletes and they can document when the athlete returns the jersey (make sure the coach is a member of athletics@chestertonannapolis.org google group).
7. Send the link also to the finance person who will be responsible for reimbursing deposit upon return of the jersey. That person should set themselves up to get a reminder email whenever anyone edits the form.
8. Create a new webpage, chestertonannapolis.org/-registration-form and use the publishing feature of jotform to get the html code for the form embedded on the new webpage and then advertise to the school where the new registration form is located.
