Request for your child's teacher to complete the Daily School Behaviour Report Card.
The daily report card targets problem behaviours in the classroom. The main areas to focus on are academic productivity, following rules, peer relationships and teacher relationships. Each day the teacher marks down how well the child meets the goals for the day. And the parent gives the reward at home. This card will foster home-school communication daily.
Instructions
Simply complete the form, add additional comments, and hit submit! We will then forward a request to complete the form. If the request is approved, you will receive an email confirmation. The Behaviour Report Card will automatically be sent to your child's teacher for completion, and the completed report will send to your email. Should you request another Behaviour Report Card, you need to submit this form again. This reporting service is limited to current registered patients of Dr Flett.
