Welcome to the STA Jotform system for therapist certification. This member profile serves as your starting point. Based on the information you complete below, you will receive an email with links to all of the other forms needed for the certification level you are working toward.

The contact information you enter here will serve as the cover page for your STA application package; all other forms in the certification process only need name and email. The confirmaiton email you'll receive upon submitting your profile will include a link to return here to update your information in the future.
