    This form MUST to be used by Club Stand-in Umpires who officate Non-Modified matches (U11-18) where an official SMJFL has not been appointed or failed to attend.

  •  If you have issued a RED card during a game you need to ring the Head of Umpiring (Doug) IMMEDIATELY after the match - 0490 495 079. In the case of a RED card you must also immediately send a photo of the official REPORT form (not the CMR) to the same mobile number so it can be dealt with by 9pm match day.

    If you issued a YELLOW card please call Doug ASAP morning with details if you believe it could possibly be escalated to a report (RED) card.
