In case of sickness, such as headaches, cramps or allergies I give permission for my youth to be administered a dose of the following basic medications:

Contract of Conduct

*I am coming to this retreat to have a great time; I realize there are rules that I must follow.

*I will refrain from inappropriate language and/or behavior. This includes but is not limited to: foul language, possession or use of illegal drugs, tobacco, alcohol, energy drinks, treating others disrespectfully, bullying, inappropriate public displays of affection towards others, etc.

*I will not, at any time, be alone with a member of the opposite sex. The breaking of this rule will mean that I will be sent home immediately at my or my Parents' expense.

*I will not, at any time, leave the premises of SGAC without the consent of my Parent(s) AND Pastor Josh with the exception of the leader supervised activity.

*I will respect SGAC property. If I break or damage something, I am financially responsible to cover the expense.

*This retreat is a privilege, not a right, and I may be sent home if I violate any of these rules or expectations.

Off-Site

I hereby give permission for my child/ren to participate in the 2023 Spring Youth Retreat on and off-site June 9 & 10, 2023. I give authorization for representatives of Spruce Grove Alliance Church to obtain necessary medical treatment as needed while off-site. I assume responsibility for any medical bills incurred, and should my child/ren be required to return home before the group for medical or disciplinary reasons, I also hereby assume any costs incurred. I give authorization for representatives of Spruce Grove Alliance Church to provide all necessary food, transportation, and lodging, as applicable. I do hereby release, forever discharge and agree to hold harmless Spruce Grove Alliance Church and the representatives thereof from any and all liability claims or demands for personal injury, sickness, or death, as well as property damage and expenses of any nature, which may be incurred by my child/ren in the course of participation in off-site activities. Furthermore, I agree to assume all responsibility for any of the previously mentioned occurrences.
