I, the parent (or guardian) of Student's Name*, do hereby give permission for my child to attend and participate in activities and programs in association with Child Evangelism Fellowship® (CEF®) for the dates of May 1st, 2024 through May 1st, 2025. These activities/programs include, but are not limited to: Christian Youth In Action®, 5-Day Club®, Camp Good News®, Good News Club®, and other CEF associated activities.
In accordance with Child Evangelism Fellowship’s Child Protection Policy, parental permission must be obtained for the following:
Waiver of Transportation. I, the guardian of the above-named minor, understand that there may be occasions when my child may be traveling from location to location in the company of only one adult of legal age. I give my permission for him/her to travel and serve with Child Evangelism Fellowship without being accompanied by two or more adults at any given time. First Initial* Last Initial*

I, the guardian of the above-named minor, grant to Child Evangelism Fellowship full use of all photographs and videos taken of the above-named minor, without reservation or limitation. I understand that the images may be used in publications, including but not limited to print publications, online publications, websites, and social media. First Initial* Last Initial*
Communications Waiver. I, the guardian of the above-named minor, give permission to Child Evangelism Fellowship and their staff to solely contact the above-named minor when contacting about CEF-affiliated programs or activities. I also give permission for photos to be emailed or texted, if in reference to a CEF-related program or activity.First Initial* Last Initial*
