ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM
I hereby assume all of the risks of participating in Beautiful Beginnings to Lasting Legacies (aka Jumping Juniors) Double Dutch Camp (“Sports Camp”), including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them, or because of their possible liability without fault.

I certify that my child is physically fit and has not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my child’s participation in this Sports Camp. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the organizers of the Sports Camp in which my child may participate and that it will govern my child’s actions and responsibilities at said Sports Camp.

In consideration of my application and permitting my child to participate in this Sports Camp, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me or my child, including traveling to and from this Sports Camp.

THE FOLLOWING ENTITIES OR PERSONS: Beautiful Beginnings to Lasting Legacies (aka Jumping Juniors) Double Dutch Camp, and/or their coaches, agents, representatives, or volunteers.

(B) I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this Sports Camp, whether caused by negligence or otherwise.

I acknowledge that this Sports Camp may carry with it the potential for death, serious injury, and personal loss. The risks may include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, actions of other people including, but not limited to, participants, volunteers, spectators, coaches, and lack of hydration.

I consent and agree that Beautiful Beginnings to Lasting Legacies (aka Jumping Juniors) Double Dutch Camp, and/or their coaches, agents, representatives, or volunteers may take photographs or digital recordings of me and/or my child as a participant during this event and use these in any and all media for training or promotional purposes. I further consent that my identity and/or my child’s identity may be revealed therein or by description text or commentary. I waive any rights, claims, or interest and I understand that there will be no financial or other remuneration.

The accident waiver, release of liability, and image release shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT ON MY OWN FREE WILL.

2026 WEBSITE AND SOCIAL MEDIA RELEASE FORM

I, the undersigned, do hereby grant permission to Beautiful Beginnings to Lasting Legacies (aka Jumping Juniors) to post my and/or my child’s story, photo, or other items, hereinafter referred to as “Materials,” that I submit to and for Beautiful Beginnings to Lasting Legacies (aka Jumping Juniors) website and social media platforms (including but not limited to X/Twitter, TikTok, Instagram, and Facebook accounts) for any legal use including but not limited to publicity, copyright purposes, illustration, advertising, and web content.

I hereby release Beautiful Beginnings to Lasting Legacies (aka Jumping Juniors), its representatives, employees, managers, members, officers, parent companies, subsidiaries, directors, and coaches from all claims and demands arising out of or in connection with any use of said “Materials”, including without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation, and any other personal and/or property rights. I acknowledge and agree that no sums whatsoever will be due to me as a result of the use and/or exploitation of the “Materials” or any rights therein.

I acknowledge that my child is under 18 years of age and lacks the legal capacity to enter into binding agreements. Accordingly, I have read this Release and consent to my child’s inclusion in the Materials and will not contest the rights granted in this Release and shall assist and support Beautiful Beginnings to Lasting Legacies (aka Jumping Juniors) in any and all legal proceedings for affirmation of this Agreement, should they choose to have a court of law affirm this.

I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT ON MY OWN FREE WILL ON BEHALF OF MY MINOR CHILD PARTICIPATING IN THIS CAMP.

By signing, I give my child permission to participate in the Jumping Juniors Double Dutch Camp, and further agree to hold harmless all volunteers, as expressed in the Release of Liability. I also commit to dropping my child off at least 5 minutes before session is set to start and being on-site to pick them up no later than 10 minutes prior to camp session ending.

In the event the I am running late, I will contact one of the coaches to inform them of my delayed arrival, or I will arrange for my child to be picked up by one of the emergency contacts (listed on the registration form).
