REignite Parent Permission Form

This form grants permission for your student to ride the van to and from the REignite on a weekly basis. The van will be supervised by approved adult volunteers/staff. All students are expected to follow safety rules and behave respectfully during transport. Any use of profanity, disrespect toward the driver or inappropriate behavior will result in immediate loss of van privilege.

If not riding the van from The HUB, students must request a ride by Wednesday at 4:30 pm by calling 419-294-1315.

A representative from WCC/The HUB will contact you weekly, on Wednesdays, to confirm your student listed above has your permission to ride in a Trinity Evangelical vehicle, to REignite, that week. Unless the WCC/The HUB representative is able to contact you, to confirm permission, your student will not be permitted to ride the Trinity vehicle that week.

Parent/Guardian Consent

I, the parent or legal guardian of the student listed above give permission for the student listed above to be transported from WCC/The HUB by an approved representative of Trinity Evangelical, in a church vehicle, for Wednesday evening activities at the church during the 2025-2026 school year. I release Trinity Evangelical and The HUB/Wyandot Community Connections, Inc (WCC) with all representatives, from any and all claims resulting from injury or damage that may be sustained by my child while participating in travel to and from REignite Student Ministry and The HUB/HUB Kids (a program of Wyandot Community Connections).

I, the undersigned parent/guardian, give permission for my child named above to ride the van provided by Trinity Evangelical Church to and from the Reignite Service each week. I understand that students will ONLY be dropped off at the address listed above, that reasonable precautions will be taken to ensure the safety of my child, and that transportation will be supervised. I also understand that if my youth has any use of profanity, disrespect toward the driver or inappropriate behavior, his/her behavior will result in immediate loss of van privilege.

I release and hold harmless the leaders, volunteers, and drivers from liability in the event of an accident or emergency during transportation. I understand that this permission is valid for the 2025–2026 school year unless revoked in writing.

I understand that use of profanity, disrespect toward the driver or inappropriate behavior will result in immediate loss of van privilege. I understand that failure to follow any of the expectations above or stated by the driver or staff may result in the loss of van privileges.
