I hereby give CM Group Holdings Inc., CM Canada Holdings Inc., CM Asia-Pacific Limited, their legal advisors, successors and assignees (hereafter “CM”) a royalty-free license to use for any purpose and/or publish through any media, including print, electronic and social, for any purpose, the photograph(s) of me, layouts, arrangements, designs, components and accompanying photographs uploaded on this form (the “Works”).
I hereby warrant that I have the authority to grant this license on behalf of all individuals appearing in the photographs featured in the Works, that I am the sole owner of the Works, that it is my original non-copyrighted work, and that I have no contractual or other arrangements which would interfere with or prevent the above-mentioned license of the Works.
I hereby waive any right that I may have to inspect or approve the use of the Works, or the use to which it may be applied. I hereby agree that any proceeds derived from the sale, publication, or use of the Works shall belong to CM.
I hereby warrant that I am of legal age and have every right to contract in my own name in the above regard, or that I am the properly appointed legal guardian of the person and property of the owner hereof. I state further that I have read the above authorization, release, and agreement prior to its execution and that I am fully familiar with the contents thereof. If I am signing as a legal guardian, I warrant that I am fully authorized to do so. This release shall be binding on the undersigned and his/her/its heirs, legal representatives, successors, and assigns.
