    Welcome to Lingfield Prep! We're so excited to have your child here.

    Please don’t look upon this as a test which your child has to pass. There are no right or wrong answers. It is simply to give us a feel for what your child can do so that we can meet their needs better when they join us.

    Try to answer the questions as honestly as you can. If you don’t know all the answers, don’t worry.

    Please let us know of anything else which you think is important and might affect your child’s learning.

    Please note that we will contact your child’s current setting for information to help support your child during the transitional settling in period.
  • Has your child attended any other Early Year’s setting prior to starting here?*

  • EYFS Statutory Guidance

    Providers must record the following information for each child in their care
  • Does your child live with both parents?*
  • Does your child have any special educational needs/additional needs/disabilities?*
  • Do they have they had an Individual Education Plan (IEP) or an Educational Health Care Plan (EHCP)?*

  • Medical information

    The following information may be needed in the event of an emergency. Please bring your child’s ‘Red Book’ with you when you come in for your first settling in session.
  • Is your child up to date with their vaccinations?*
  • Does your child need medication administered on a daily basis?*
  • Please note that in an emergency, our staff may be asked to provide information to NHS staff regarding your child's medical history. I give permission for staff at Lingfield College to seek any necessary emergency medical advice or treatment and to discuss my child's medical history if needed*

  • Information Sharing

    It may sometimes be helpful to share information about your child's development with other agencies/settings, particularly at the time of transition. You will always be informed when this is happening and who with.
