This form is for organizations that are seeking a certified trainer to deliver the Ontario Smart Serve training program. Please complete the information below and we will get back to you with recommendations for a trainer in your area.

Important: People attending training and certification MUST provide government issued photo identification.

People attending the training will need a computer/laptop with a web camera. This is required to write the Smart Serve Ontario certification exam.

Pricing:

Trainers are billed at CAD$1,500 per session.

Each participant will be billed for the Smart Serve certification exam fee of $45.

Travel expenses may be in addition depending on the distance from the trainer.

If we have to secure a training venue, that cost will be added to the final invoice.

All fees are subject to HST.

Once a trainer is confirmed a deposit of CAD$1,000 is required to secure the training.
