家长或监护人同意书 Informed Consent and Acknowledgement

1、注册费不可退还。

2、按月付学费需要在每月第一个工作日付清。超过截止日期付学费，会收取$25/周的滞纳金。

3、学生因故不能来上课，学费将不退还，除非因老师个人或学校安排等原因无法提供课程。如果中途退出，将按照$200/月计算实际应交学费，然后从已交学费中减去，余额即为退还金额。

1. The registration fee is non-refundable.

2. The monthly payment is due on the first workday of each month. Late payment fee will be charged at the rate of $25/week.

3. Unless the class is canceled because of the teacher's personal reason or school arrangement, the tuition will NOT be refunded if students miss the class. The actual attendance will be calculated at the rate of $200/month, and will be deducted from the whole year tuition for a refund.

We may need to record the class or activities just in case students miss the class so they can watch it later and catch up.
