    Registration form below is for all football/cheer for SDQC for the 2024 season. This form allows for up to three athletes registered at one time. For more than three athletes, please contact the SDQC at stuartsdraftqc.va@gmail.com.
  • ****Online payments cost the SDQC additional bank and credit card fees. Registration fees charged will go further when you help us cover the cost.

    All online orders will be charged 4% to help cover those costs.
    There will be no refunds except for individual exceptions approved by the SDQC Board. Credit card fees are non-refundable regardless.

    Otherwise, in-person registrations by cash or check will be accepted April 6, May 4, June 8 and July 18. Thank you for supporting the SDQC!!!

  • PLEASE READ THE FOLLOWING BEFORE SUBMITTING:

    I/We, the parents or guardians of the above named youth, do hereby give my/our approval for his/her participation in any activities of this sport. By doing so, I/we assume all risks and hazards incidental to such participation and transportation to and from said events. I/We do further waive, release, absolve, indemnify, and agree to hold harmless the Augusta County Quarterback Club, Inc., Stuarts Draft Quarterback Club, organizers, sponsors, supervisors, and other participants, and persons transporting my/our child to and from any form of the above selected sport and/or activities.

    In the event of an accident, when I/we cannot be contacted, I/we give my/our permission to call 911 and take my/our child to the emergency room at the nearest hospital if needed, and the hospital and its medical staff has my/our authority to provide treatment which a physician deems necessary for the well-being of my/our child.

    I/We do hereby agree to return any equipment issued to my/our child for his/her participation at the end of the season. Any equipment not returned in good condition other than normal wear, will be my/our financial responsibility to pay the replacement cost. I/We further understand that by not returning equipment in said condition, deposits will be required for future participation, and legal action and its cost can and will be taken against me/us through Small Claims Court.

    It is further understood that each area’s registration fee will be used for expenses incurred in continuance of said sport and should not be considered as right of ownership of equipment.

    Regarding COMPETITION CHEER: Mandatory Sunday practices are required. Multiple absences (excused or unexcused) will result in immediate dismissal from the competition team.  

    I/we also agree I/we will volunteer our time as needed to work the following: concession stand, chain gang, time clock, spotter, announcer, and/or trash pick-up. Additionally, by my/our signature below, I/we agree that the I/we will abide by the Sport Parent Code of Conduct.

    We also agree that we will review the SDQC Participant Rules of Conduct with the participant for whom we are responsible, and that the participant will abide by those rules. Submission of this form agrees to that Participant Rules of conduct. The SDQC emphasizes fundamentals, teamwork, fair play, cooperation, and respect for the officials, one’s self, one’s teammates, and the other team. I hereby pledge to provide positive support, care, and encouragement for my child participating in the SDQC program by following this Parent Code of Conduct: I will encourage good sportsmanship by demonstrating positive support for all participants, coaches, and officials. I will do my best to make this program fun for my child. (A child usually forgets the outcome of the game: it is the adults that have a tendency to stress the win/loss record.) I will ask my child to treat other participants, coaches, officials, parents, and spectators with respect regardless, of race, age, sex, or ability. I will treat officials, volunteers and coaches with respect. They are there to provide my child/children with a safe and healthy learning environment. Officials, volunteers and coaches deserve the same type of environment from the parents and spectators and I will abide by these terms. I recognize the importance of volunteer coaches and volunteer board members and offer to volunteer my time if needed in concessions or in the press box etc. I will communicate with the coaches and board members and support their decisions. If I have an issue I will allow a 24 hour window before lodging a complaint and will not become irate and cause a scene on game day, during events or during practice. I will follow the 24 hour rule, to allow time to decompress my thoughts and enter a conversation with clear thoughts and follow the proper chain of commands (coordinators first, board members second, coordinators, board members and coaches third) to resolve any issues that I may have. I understand that the parent conduct rules go for all of my family, friends, legal guardians, co-workers that may attend practice or games. I understand that I take responsibility and will hold them accountable to conduct themselves properly. Failure to conduct myself may result in suspension (these are also outlined in the SDQC bi-laws, if you need a copy please ask for one) SDQC does have the right to ban individuals for behavior and conduct.

    By submitting this you will be agreeing and submitting for spouses, relatives, friends, and or guardians:

    I agree to volunteer for at least one home game shift or find someone as my substitute.

    I have read and agree to the expectations for parent conduct and understand the consequences of non compliance with these expectations. 


    Registration fees are NONREFUNDABLE ONCE SEASON STARTS.
