• Pet Travel Form

    This form is intended for existing Grand Lake Veterinary Hospital clients traveling with a pet.
  • Providing all of the following information prior to your pet’s appointment will help to expedite the necessary paperwork for travel.

    Please complete one form per pet.

  • Not sure where to start? Complete the first 3 steps below to prepare, then complete the form!

    1.  Check with both USDA APHIS Pet Travel and your airline to find out travel requirements and documentation needed.
      As the owner, you must be familiar with travel guidelines and that import permits, blood testing, vaccinations and other treatments may be required.  
    2. Check  APHIS Veterinary Services Endorsement Offices to see if you will need your certificate to be endorsed by the USDA. Once you obtain all required documentation and treatments, the owner may need to obtain a seal of endorsement via SFO or Sacramento USDA Veterinarian.
    3. Obtain a signed copy of your pet’s rabies certificate. If your pet’s recent rabies vaccination was given at GLVH, we will provide a copy to include with the health certificate.

  • If traveling to Hawai`i, your dog or cat must have been vaccinated at least twice for rabies in its lifetime. The rabies vaccines must have been administered more than 30 days apart. The current (most recent) rabies vaccine must have been administered more than 30 days before the pet’s date of arrival in Hawai`i.

    Please list when the last two rabies vaccines were adminsitered and at what facility. If your pet has not been vaccinated or has only had one vaccination, please indicate.

It is your sole obligation to obtain proper forms, documentations, medications, vaccinations, pet identification and endorsement by the USDA or other government entities. You are aware that regulations may vary depending on travel destination and that they are subject to change at any time.
