Great North Woods Pool Access Request and Liability Release

PLEASE READ THE CONTENTS OF THIS FORM CAREFULLY BEFORE SUBMITTING 

Current residents and owners of units at the Great North Woods Condominiums may request access to the community swimming pool by making an access device selection below and providing the required information. Users under 18 cannot request access devices themselves; the request must be made by an adult head of household.  After the form is submitted, a copy will be emailed to the email address provided (you may not receive the email immediately, but within 24 hours, and check your SPAM folder!!) and to the Great North Woods Pool Administrator who will issue your requested device(s) or contact you for further information. Please allow 5-7 business days for processing requests.  Your device(s) will be delivered either in person or via mail to the address provided on this form.

I hereby acknowledge and declare that:

Submitting this request and receiving access to the Great North Woods Condominiums Swimming Pool constitutes acceptance of the Great North Woods Condominium Association Pool Regulations.  Pool access and activities may be monitored and recorded. Violations of Pool Regulations (or any Association rules) by any pool user may result in temporary or permanent deactivation of access devices and possible fines, at the full discretion of management.   

I agree to indemnify, release, and hold harmless the Great North Woods Condominium Association, its board members, property management, employees, agents, and representatives from any cost, liability, claim, damages, demand arising from injury, loss, illness, or death to me, my immediate family, guests, invitees related to the use of the pool. Likewise, I agree to hold the Great North Woods Condominium Association,  its board members, property management, employees, agents, and representatives harmless for any damage or loss to personal property caused by or related to the use of the Great North Woods Condominium Association Swimming Pool or its facilities.

I am of legal age and fully understand and accept the information above, and I agree to be held legally accountable for any and all effects of this Release.
