IMPORTANT PLEASE READ FULLY AND ACCEPT THESE TERMS BEFORE CONTINUING

I/We confirm by signing this form that I/We are physically sound and that I/We have sought approval if required, to proceed with a routine of exercise.

I/We confirm that all strength training, cardiovascular exercise, or any other exercise shall be undertaken by me at my sole risk and that Royal Norwich Golf Club Limited or any of its employees/contractors shall not be liable to me for claims, demands, injuries, damages, actions or causes of action, whatsoever, to my person or property arising out of or connected with the use by me of the services provided and of the premises where the same is located. I/We do hereby expressly forever release and discharge Royal Norwich Golf Club Limited and its employees/contractors from all such claims, demands, injuries, damages, actions or causes of action, from all acts of passive negligence on the part of Royal Norwich Golf Club Limited and its employees/contractors.

I/We further expressly agree that I/We will not use equipment improperly. If I/We have any questions whatsoever, concerning exercise and use of equipment, I/We agree that I/We will request instruction from Royal Norwich Golf Club Limited or its employees/contractors.

I/We understand that Royal Norwich Golf Club Limited does not provide, supervision, instruction or assistance for the use of the facilities and equipment.

I/We agree to conduct ourselves in a controlled and reasonable manner at all times, and to refrain from using any equipment in a manner inconsistent with its intended design and purpose. I/We understand that there is no panic button or immediate alert system should there be a problem in the gym.

I/We will be mindful of others around me and will alert Royal Norwich Golf Club Limited or the emergency services if I/We have concerns about myself or anyone else whilst using the gym.

I/We agree, that, in the event of this application being successful, I/We will at all times conform to the Rules, Bye-Laws, Articles of Association and Members Charter of Royal Norwich Golf Club Ltd, and to such amendments as may from time to time be promulgated and that personal details be held on the Club Database in accordance with the GDPR 2018.

We use the information above and below to allow us to fulfil our contractual obligations to you as a member to enable us to send you an annual subscription renewal and also to process competitions. We share this information with our IT systems but your personal data is administered internally by the club’s data controller who adheres to the Golf Club’s internal data protection policy and procedures. We do not sell or pass on data to external companies for marketing purposes.

We are also able to hold further information to be able to correspond with you regarding our club activities including events and competitions by way of post, telephone, email or text.

We may also share your information with members of the club, as a legitimate interest under the General Data Protection Regulations (“GDPR”) 2018, so that they can contact you direct to arrange golfing related matters.

[1] Please note if you do not select any of the communication options, we will not contact you relating to club activities and it will rely on you visiting the club to keep up to date with events and news. However, you will still receive club information that we are required to send in order to fulfil our contract with you. Some of the golf booking and finance system functions preferred contact is by email.
