    • Stars At Sea does not collect individual deposits from guests and is not responsible for forwarding this form or ensuring your group leader receives your information or payment.
    • Stars At Sea does not track or reconcile individual form submissions with deposits received by your group leader.
    • Failure to submit this form and deposit to your group leader within the required timeframe may result in loss of space, pricing, or eligibility for the program.
    • Deposit amounts are determined by your group leader based on room occupancy and group requirements. Incorrect or incomplete deposits will result in your reservation not being processed until corrected.
    • If you are interested in upgrading to a suite, select “suite” from the room request menu. Suite availability and pricing will be provided separately. If unavailable, a balcony stateroom will be reserved.

    By submitting this registration, I confirm that I have read, understand, and agree to the Stars At Sea Guest Terms & Conditions (www.starsatsea.com/terms). I confirm that I am authorized to submit this registration on behalf of all guests included in this reservation, including any minors where applicable, and that I am responsible for communicating these terms to all participants and, where applicable, their parent or legal guardian. I acknowledge that this reservation is subject to cancellation penalties imposed by the Travel Supplier and Stars At Sea, which may be up to 100% of the total booking value depending on the timing of cancellation; that Stars At Sea acts solely as a coordinator and is not responsible for changes made by the Travel Supplier, including but not limited to itinerary, scheduling, or onboard program changes; that no reservation exists until my deposit is received and processed by the Travel Supplier, at which point cancellation penalties will apply; and that my group leader is authorized to approve and coordinate all booking-related decisions on behalf of the group. I agree to be bound by these terms whether or not I have separately reviewed the full Terms & Conditions.
