Activity and Transportation Release
I give permission for my child to participate in activities with the Circle of Friends program. I also give permission for my child to be transported in the automobile of Circle of Friends volunteers and staff for these activities. I agree not to hold Circle of Friends program, its staff, and volunteers liable for any injury to my child while they are participating in this program.
