  • The next part of this application is for a brief review. The review is a part of the application process because in order to better serve the community, the Theus Family Foundation must know if we are doing our best to be a help to the community. We want to make helping the community an easy process and your review of this application will help to make that happen. Your honest review will help us to improve in areas where we may not be progressing. This review has no bearing on the outcome of this application.

If the applicant did not upload their documentation online when the application is turned in, a staff member will reach out to the applicant to make an appointment to bring in proof of residency and proof of income. Also for a quicker application process you may email the proof of residency and proof of income to: tff@theusff.net or fax it to 731-318-5540. When all paperwork have been reviewed and studied a staff member will call or email the applicant for either an approval, denial or for more information that is needed.
