Tuscaloosa Academy Consent and Waiver Form
In consideration of being permitted by Tuscaloosa Academy to participate in activities, events, and programs, including but not limited to athletics, field trips, and extracurricular activities, I, the undersigned, hereby acknowledge, agree to, and represent as follows:
Assumption of Risk: I understand that participation in activities, events, and programs may involve certain risks, including but not limited to bodily injury, property damage, illness, or even death. I voluntarily assume all such risks, both known and unknown, and acknowledge that my participation is solely at my own risk.
Health and Medical Conditions: I certify that I am physically fit to participate in the activities, events, and programs provided by Tuscaloosa Academy. I agree to disclose any pre-existing medical conditions, allergies, or other health-related concerns that may affect my participation. I authorize Tuscaloosa Academy staff to obtain medical treatment for me in the event of an emergency if I am unable to communicate my consent.
Photography and Publicity: I consent to the use of my likeness, voice, and/or image in photographs, videos, and other media recordings captured during my participation in Tuscaloosa Academy activities, events, and programs. I understand that these materials may be used for promotional and publicity purposes by Tuscaloosa Academy and its affiliates.
Release and Waiver of Liability: In consideration of being permitted to participate in Tuscaloosa Academy activities, events, and programs, I hereby release, waive, discharge, and covenant not to sue Tuscaloosa Academy, its trustees, officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including but not limited to bodily injury, property damage, or death, that may be sustained by me or any property belonging to me while participating in such activities, events, or programs.
Indemnification: I agree to indemnify and hold harmless Tuscaloosa Academy, its trustees, officers, employees, agents, and volunteers from any and all liabilities, damages, costs, or expenses, including reasonable attorneys' fees, arising out of or related to my participation in Tuscaloosa Academy activities, events, and programs.
Governing Law: This Consent and Waiver Form shall be governed by and construed in accordance with the laws of the State of Alabama.
I acknowledge that I have carefully read and fully understand the contents of this Consent and Waiver Form, and I voluntarily agree to its terms and conditions.
