I, for myself, and my representatives, heirs, and assigns, hereby completely release and forever discharge Sprout City Farms and the City of Lakewood, and its present, past and future predecessors, successors and affiliated and related companies and instrumentalities, and their respective partners, shareholders, commissioners, directors, officers, employees, agents, insurers and attorneys (collectively referred to hereafter as the "Released Parties"), of and from, and promise not to sue or assert against the Released Parties for any purpose, any and all claims, actions, causes of action, losses, liabilities and demands for damages of any kind, irrespective of the theory upon which they may be based, including but not limited to general, special or punitive damages, attorneys' fees, expenses, or other compensation, which in any way relate to or arise out of any and all events or transactions relating to volunteering at the Mountair Park Community Farm or otherwise, which I may now have or hereafter have, whether known or unknown, foreseen or unforeseen, suspected or unsuspected, in law or equity.

I am over cighteen (18) years of age. If under 18, a parent or guardian must sign this form on behalf of the minor.

Part 3: Authorization to Use Photograph

Iagree and consent that Sprout City Farms and the City of Lakewood may copyright and/or use and/or publish any motion pictures, still photographs, videotape recordings, magnetic tape recordings, and written materials, made or taken during this event, in any form, style, or color, together with any writing and/or other advertising and/or publicity material in connection therewith, including use of my name, as they may select.Over

This consent and release is given by mc without limitations upon any use for projection, playback, reprints, rerun, broadcast, telecast, internet or publication of any kind, including advertising and publicity connected therewith. I hereby waive any right that I may have to inspect and approve the finished product or copy that may be used in connection with an image that has been taken of me, or the use to which it may beapplied. I hereby acknowledge that Sprout City Farms and the City of Lakewood will own all rights to the photographs irrespective of the form or media into which they are produced or used.
