    AxisCare Policy

    We have a new computer software system that we will be using, effective immediately. You must be using this software to clock in and out on. Caregivers are to IMMEDIATELY notify the office personnel with any issues that may arise so that it can be fixed. This will be used In continuation of paper billing that is still do on Saturday Night by Midnight.

    Please notify office immediately so that we can fix any issues that arise and we can compare your paper billing with the software where the caregiver clocking on and out on. All times and task must match on application and service records. Any discrepancies will not be send to HR for billing, but the caregiver will be notified and the situation will be corrected. Any discrepancies or delay In billing being turned in could result In a delay in getting payroll checks cut and an Interruption in your pay. We are making every effort to avoid this, but the caregiver has to complete everything and have it turned in to the office In the designated areas In a timely manner.

    There is a telephony system that can be used to call to clock in and out on if you do not havea phone that supports the application being downloaded on, but you have to notify the office immediately to get this set up. We are all in this together, but the caregivers MUST do their part.

    This is something that we are required by the state to do and we have no choice as a company to be in compliance with the rules and regulations. Not compliance with this, can result In disciplinary actions, including termination of caregiver contracts.

    Please remember to turn in your paper service logs as usual as we work on getting everything in the system running as it needs to run. We will notify caregiver if/when we stop using paper service records all together. At this time, we do not have a date set to stop paper service records to be used along with AxdsCare.

You are also stating that you have received a copy of this policy and you are held responsible for staying in compliance with this policy.

    We are now using AxisCare as our portal for billing. This means that you will be clocking in and out on the app starting immediately. You will also continue to do paper billing and turning it In by Saturday Night at midnight so that we can verify that the computer system and your paper billing match so that we can process your check each week. Please be mindful to make sure that your times in and out on each client match in the computer system and on your billing sheets that you turn in, We also need the marked task to match so that billing can be verified. You are to be reporting any issues or concerns to your care coordinator Immediately so that we can fix any issues that you may be having. Elite Senior Care Management Company LLC is holding you accountable for doing your job and doing it correctly, however, we will assist you with any issues that arise. If you do not properly do the above mentioned, this could result in your paychecks not being processed and hinder your paychecks from being able to be cut and delivered in a timely manner.     With ESC live in Axis Care, it is required that you have clock in and out or telephony in and out. We must have an active phone number in the system into which you are doing this from. If you do not have an active phone, we will not be able to assign you clients. You are required as contractors to supply all necessities to perform your job function. If you have an issue with having a phone, I recommend that you buy a cheap phone with minutes from a store to use solely for work, or that you stop at one of the tents giving out free government phones and see if you qualify.

    Also, remember that we must have your active number in the system before you can use that phone. This means that anytime you switch phone numbers, you must let us know immediately so that we can correct the system so there is no issue with clocking in and out.

  • COMMUNICABLE DISEASE/INFECTION CONTROL POLICY

    The purpose of this policy is the prevention of the spread of infectious and communicable disease from agency staff and/or individuals providing services to clients. (ADH, 10-2 Per ADH Licensure, work restrictions will be placed on employees and/or contracted individuals who are known to be affected with any disease in a communicable state or to be a carrier of such disease, to be affected with boils, infected wounds, diarrhea or acute respiratory infections. Anyone with the named above conditions will not be allowed to work in any area in any capacity in which there is the likelihood of transmitting disease to clients, agency personnel or other individuals within the home.

    All contractors who report a communicable disease must be cleared by a physician before being allowed to return/start work.

  • Importance of plan of care

    The plan of care is important because it is written orders from the patient's physician and/or DAAS RN. This gives the caregiver or nurse etc. what the doctor has ordered and the written plan how to take care of the patient.

    3. Notify RN first if you notice a change in patient. 4. Always follow HIPAA rules. Plan of care is confidential, as well as any information in patient chart. Keep plan of care in folder and do not leave it open where anyone could read it. 5. If you have any questions on plan of care-call ESC.

  • Purpose & Philosophy of AR Choices Waiver Program

    To qualify for the AR Choices Program, a person must be age 21 through 64 and who are determined to have a physical disability through the Social Security Administration or the Department of Human Services (DHS) Medical Review Team (MRT) and require an intermediate level of care in a nursing facility, or be 65 years of age or older and require an intermediate level of care in a nursing facility. Persons determined to meet the skilled level of care, as determined by the Office of Long Term Care, are not eligible for the AR Choices Program. Medicaid eligibility is determined by the DHS County Office, and the DAAS Long- Term Services and Supports (LTSS) Program Eligibility Specialist and is based on non- functional and functional criteria. The individual must be an individual with a functional need. To be determined an individual with a functional need; an individual must meet at least one of the following three criteria, as determined by a licensed medical professional: The individual is unable to perform either of the following: At least 1 of the 3 activities of daily living (ADLs) of transferring/locomotion, a. eating or toileting without extensive assistance from, or total dependence upon another person; or b.At least 2 of the 3 ADLs of transferring/locomotion, eating, or toileting without limited assistance from another person.

  • Recordkeeping & Reporting Condition Change Policy

    ESC policy for recordkeeping: 1. All Caregivers will be instructed to follow the plan of care. 2. All Caregivers will be instructed to complete all charting correctly, legibly, and honestly. All Caregivers are aware of the disciplinary actions that will be taken if any fraudulent activities are being committed. 3. All Caregivers will be instructed to have each client sign and verify date of service, time in and out. 4. All Caregivers will Report to the RN of any change in client. 5. All Caregivers will follow the HIPAA policies on charting. Keep all paperwork confidential. Keep all paperwork attended, not left in unlocked vehicles. 6. When new policies on charting are put in effect, all Caregivers will be instructed or trained regarding the change or addition and will implement new policies into charting. 7. All Caregiver charting will be supervised by RN supervisor, and updated, monitored, and notified of any errors in charting. If any errors in charting are found, RN will document, and give corrective information to correct the error, and will follow up with monitoring and document that corrective measures were or were not successful and will follow policy per outcome of review. 8.All Caregivers will document on the back of the service record on any notices that are necessary, such as if patient is not at home etc., or if for any reason services were not provided. If patient is in hospital Caregiver must notify the office. 9. When you notice a change in the clients condition, you must report this to the RN, document on the back of the service record that you reported client condition to the RN. The RN will document in the Client chart and take the necessary measures needed.

  • Sexual Harrasment Policy

    Elite will not tolerate any sexual harassment by workers or clients.

    Any sexual harassment should be reported immediately to the supervising RN. The aide/client that is allegedly being inappropriate will be interviewed. A report will be filed, and Elite will take proper actions to correct the incident. If the accusation is regarding a contractor, the contract will be suspended until the complaint is resolved. If the contractor is found guilty, the contract will be terminated and Adult Protective Services will be notified, along with the DHHS RN if applicable.

    If the accusation is regarding a client, the client will be placed on hold until the complaint is resolved. If the client is found guilty, the primary care physician will be notified, the DHHS RN will be notified if applicable. The client will be warned and a more appropriate caregiver for the client will be placed.

    If the accusation is against the office staff, that person will be interviewed. Any accusations will be reported and documented along with the actions taken to correct the problem. If the person is found guilty, the director of the agency and/or the RN supervisor will take the appropriate actions.

    Any worker that is proven to be guilty of sexual harassment will be terminated.

    Any client who, after being written and warned that continues to violate the sexual harassment policy will be terminated of services and given a list of other agencies that they may choose from.

    Any worker or client that feels that they were unfairly accused has the right to appeal they findings and or file a grievance, all supervisor and grievance dept., numbers and contacts are available upon request.

  • Attention Caregivers:

    You are not allowed to have anyone else in the vehicle with you while you are transporting a client. Everyone who is in the vehicle with a client, must have a state/federal background check and a Child and Adult Maltreatment Central Registry check ran through Elite Senior Care, LLC to be around the clients. Anyone who is caught not complying with this, will have their contracts terminated immediately. This is not an acceptable practice and will not be tolerated. Also, this is considered a breach of HIPAA, and there is a whole other set of civil and criminal penalties that go along with that. To comply with all state and federal laws, we must make sure that we are educating our caregivers on the appropriate rules and regulations. If you have any questions, please feel free to ask any of the office staff. There are no exceptions to this rule. This includes not transporting more than one client in your vehicle at a time.

    Elite Senior Care follows the Office of Long Term Care (OLTC) guidelines and instructions. The manual is kept in the RN Supervisor's office and is available for viewing at any time. The RN will go over the procedures step by step with the training curriculum if further assistance or training is needed on any task.

  • IN-SERVICE POLICY/CONTINUING EDUCATION

    Effective continuing education is crucial for professional growth. Medicaid

    requires personal care aides to participate in at least twelve (12) hours on in-service training every twelve (12) months. Each in-service training session must be at least one hour in length

    or self-study. Elite is mandated to maintain documentation that all contract workers are

    meeting the in-service requirement. Elite provides the continuing education free of charge to you. However, it is your responsibility to maintain your hours. If you don't complete the mandatory amount, you will not be allowed to continue services to our clients. You must make

    an 80% on each CE to pass. If you need assistance with the test or material, please schedule an appointment with the supervising RN. The RN will go over the information with you.

  • Code of Ethics Policy

    Arkansas Department of Human Services (DHS) and the Arkansas Department of Health (ADH) require contracted workers providing services to clients to adhere to the following code of ethics:

    No consumption of the client's food or drinks.

    No use of the client's telephone for personal calls.

    No discussion of one's personal problems, religious or political beliefs with the client. No acceptance of gifts or tips from the client or their family members. No friends, relatives or unauthorized individuals are to accompany the contract worker to

    No consumption of alcoholic beverages or use on non-prescribed drugs prior to service

    delivery nor in the client's home.

    No smoking in the client's home (even if they do)

    No solicitation of money or goods from the client No breech of the client's privacy or confidentiality of records.

  • HIPAA Confidentiality Agreement

    Contractors will have access to confidential information, both written and oral, during your contract and

    job responsibilities. It is important that this information is not disclosed to any unauthorized individuals

    to maintain the integrity of the client information. An unauthorized individual would be any person that is not currently a contract worker assigned to the client and/or office staff of the agency. Any other disclosures may only occur at the direction of the privacy in the office or by patient authorization Violations could result in legal ramifications for all parties involved. Make sure that you understand the agency's policies with regards to privacy and security of personal health information. You are agreeing to maintain confidentiality of all information obtained during contract including, but not limited to: financial, personal, and sensitive information regarding clients, contract workers, employees. Understand that inappropriate disclosure or release of client information is grounds for termination

    of contract. Ex: Your clients should not know the name of your other clients.

  • DISCIPLINARY ACTIONS AND PROCEDURES POLICY

    It is ESC policy on discipline as follows: 1. 1st offense, depending on severity, will be verbally warned or reprimanded. A written complaint form will be filled out, signed, and dated by both the contract worker and supervisor. This will be kept in a disciplinary file. 2. 2nd offense for same offense, depending on the severity, may result in termination. If termination of contract worker results, ESC will follow the policy and procedures for termination. 3. If a disciplinary action is taken due to a client complaint, ESC will follow policy on the client complaint form and keep signed record in disciplinary and complaint files. 4. All disciplinary actions will be done in a professional, private manner. All actions will be documented and recorded in a disciplinary file. 5. ESC reserves the right to terminate a contract worker at any time, rather first offense or not. It is contingent upon the severity of the complaint. All contract worker/clients are given the Right to File a Grievance form upon signing on.
