PHTA accepts change requests for any PHTA Standard at any time.  Change requests for standards that are not currently in development will be held until the revision cycle starts for each standard.

By completing this form, you represent and warrant that you are the sole owner of any intellectual property (including images or diagrams) that you are submitting and that any proposed language you are submitting is not the property of any other individual or entity, and that you hereby grant PHTA all and full rights in copyright for any proposed language or information (including images or diagrams) that you provide the committee and understand that you have no rights in any publication of PHTA that uses this information in any form.
