Summer Camp and Work Based Learning Experience Agreement
The Summer Camp will take place from June 5th - June 23rd. During this summer camp we will focus on pre-employment skills, daily outings from campus tours to community integration activities. We will also practice daily living skills, orientation and mobility as well as technology. The Work Based Learning Experience (WBLE) will take place from July 10th - July 31st. Each student is given a job placement within the community where they are given the opportunity to build and enhance their skills.
Please read through the agreement and sign below:
- To help improve parent / participant communication, we understand that all email and phone numbers must be up to date
- Teen will actively participate throughout the summer camp and WBLE (at least 80% attendance)
- During summer camp and WBLE, teens will be engaged and respectful during all activities at Lighthouse and in the community
- During WBLE, teens will fulfill all duties, be on time, dress accordingly and exhibit professionalism
- If teen is unable to attend a session throughout the summer, parent and/or student must communicate either via email or phone at least 48 hours in advance (unless sick/emergency)
- Teens must take more initiative and responsibility during their work experience. The program is aligned to support independence and accountability for participants to increase their overall independence skills
It is the student and parent/guardian responsibility to read, discuss, and understand the information,rules and procedures.
