The Rexdale Soccer League is introducing the Volunteer Screening Initiative as required by the Ontario Soccer Association. Please list the names and phone numbers of 3 references we may contact with respect to your application to coach. No personal references please; all references must be from a community organization, previous and/or current place of business or educational setting.

I agree to abide by the Constitution and the By-Laws, Rules, and Regulations of the Rexdale Soccer League and obtain a police reference check as necessary to fulfill my capacity as a volunteer coach this season. I will respect all members of the league and do my best to ensure that all members of the League under my supervision adhere to the objective and rules of the Rexdale Soccer League. The Rexdale Soccer League will not be held responsible or liable for any unforeseen circumstances beyond its directed control, including any willful action(s) I may undertake that result in damage, loss or contravention of the laws of the province of Ontario.

I understand, commit and dedicate service to the Rexdale Soccer League, community, and players in a volunteer capacity.  My conduct, deportment, and actions align with the Rexdale Soccer values, and I will support the program for the entire season.

Note: Upon successful completion of volunteering for the entire season, a $25 refund will be issued to offset the reference check fee.
