  • This agreement is made on this   Pick a Date   by and between German Shepherd Dog Rescue of Georgia, inc. (GSDRGA) and         herein referred to as Adopter.

  • In consideration of a monetary donation in the sum of  $   the receipt of which is hereby acknowledged, the possession and ownership of the above
    mentioned German Shepherd Dog (GSD), will transfer from GSDRGA to the Adopter herein. The Adopter does hereby agree to abide by the Terms of Adoption as outlined below.

    1.  I agree to keep an identification tag attached to a properly fitted collar which will remain on the GSD at all times, whether inside or outside the house. I will obtain
      any licenses as required by local authorities.
    2. I agree to provide the GSD with all necessary preventive and emergency care including: vaccinations, heartworm preventive, dietary prescriptions, wound or bone/muscle/tendon afflictions at the intervals advised by my veterinarian.
    3. I agree to have the GSD under my control when it is not on the confines of my property. The GSD will not be confined by being tied or chained. I will keep the GSD as a family member and companion and not as a kennel dog. For the safety of the dog, I will never permit the GSD to roam free in an unfenced area without direct adult supervision.
    4. I agree not to abuse or neglect the GSD and I authorize GSDRGA at their discretion, to determine whether or not the GSD has been abused or neglected.  I shall under no circumstances physically punish the GSD. Rather I will consult with GSDRGA, my veterinarian or trainer about the appropriate ways of dealing with specific behavioral problems. The Adopter will refrain from physical abuse and other inhumane treatment and will prevent other adults and children from
      teasing or physically abusing the GSD.
    5. I understand I will only euthanize the GSD for an untreatable medical condition, as attested to by a licensed veterinarian, with written notification submitted to GSDRGA. If the Adopter feels it necessary to consider euthanizing the GSD without the diagnosis of an untreatable medical condition, the adopter agrees to contact GSDRGA for consultation prior to choosing this option. “Euthanizing” the
      dog means intentionally putting the animal to death by any method including gas and lethal injection, otherwise known as “putting to sleep”.
    6. I understand the GSD covered by these adoption papers is as far as can be determined by GSDRGA in good health unless otherwise specified. GSDRGA is providing this GSD for adoption only after an initial health check, required vaccinations and sterilization by a veterinarian. Understanding this, GSDRGA
      cannot guarantee the health of the GSD and will not be responsible for any medical fees incurred after the adoption date. If a health problem does develops during the first 10 (ten) days, the adopter will notify GSDRGA of the situation.  The Adopter assumes all responsibility for treatment of any and all existing conditions and those that may subsequently occur. If the GSD has a known
      medical problem it will be listed as Addendum to this agreement. By signing this agreement, the Adopter acknowledges the understanding of the extent of the medical problems and the recommended treatment as specified in the
      Addendum.

    7. GSDRGA has made every effort to  evaluate the temperament of the GSD in a variety of social situations. Knowing this, I understand GSDRGA makes no explicit guarantees or commitments regarding the temperament of the GSD. The GSD is adopted “as is” and the Adopter assumes all responsibility for any and all existing temperament conditions that may occur. GSDRGA will not be held responsible for the temperament of the GSD or how the GSD will react in certain situations. Likewise, GSDRGA cannot guarantee that the dog is housebroken or will remain housebroken in every environmental
      circumstance.
    8. I agree to provide notification to GSDRGA within 14 (fourteen) days of any change of address, phone number or any change of circumstances that will affect the GSD, as well as notice of any incident involving police, animal control  or SPCA.
    9. Regarding transportation of the GSD, I agree not to permit the GSD:
      1. To ride in the back of an open pickup truck other than in a secured crate
      2. To remain unattended in any vehicle at any time
      3. To be flown in an airplane without pressurized and climate controlled hold
        areas or when the temperature at either departure or landing is above is 60 or below 45 degrees.
    10. I agree to allow GSDRGA reasonable access to the GSD to assure that it has integrated to its new environment and its new family and to ensure the compliance with the terms of this agreement.
    11. I agree the GSD is not to be left unattended with any young children at ANY time.
    12. I agree that during the lifetime of the GSD, GSDRGA retains the right to reclaim the GSD at any time because of neglect, abuse, inadequate care, breach of this agreement, or lack of integration into the Adopter’s family as determined by GSDRGA. The adopter agrees to relinquish custody of the GSD immediately
      upon request due to any of the above mentioned circumstances. In the event that such action is necessary the Adopter hereby waives any claim for a refund of any kind for any fees and expenses that may have been incurred.
    13. I agree that I will not transfer ownership of the GSD to any other person or entity without the acknowledgement of GSDRGA. The Adopter agrees to notify GSDRGA within 14 (fourteen) days in the event of the death of the GSD. In addition, the Adopter agrees to notify GSDRGA if the GSD becomes lost for more than 24 hours. GSDRGA may be able to assist in the search.
    14. I understand that GSDRGA will provide lifetime support either in person, by phone or by email to assist with integration of the GSD into the adopter’s family and to support the general well-being of the GSD throughout its entire life. If GSDRGA feels the Adopter has made every effort to integrate the GSD into their family but remains unable to
      develop a bond with the GSD, upon approval of the board of directors, GSDRGA will accept the GSD back into the rescue during the first 30
      (thirty) days after the date of adoption. If the GSD is returned in good health and any equipment provided is returned clean and undamaged, GSDRGA will return to the Adopter 50% or half of the adoption
      donation, if the Adopter so requests.
  • I understand that any failure to perform and honor the foregoing agreement will constitute a breach of this agreement. In the event of any such breach, I authorize and allow GSDRGA to reclaim both possession and ownership of the GSD. I will at my own expense return the GSD back to GSDRGA or in the alternative allow a GSDRGA representative to reclaim the GSD from me. The Adopter waives any claim for refund of any kind for any fees and/or expenses the Adopter may have incurred.

    The Adopter does hereby understand, promise, and agree to forever release, discharge and covenant to hold harmless GSDRGA from any and all claims, damages, costs and expenses, loss of services, actions and causes of action belonging to the adopter, arising out of adoption or the actions of the GSD.

    DISCLAIMER: GSDRGA gives no warranty, expressed or implied, as to the description of
    the GSD, quality, personality, health, temperament, physical condition and suitability for any purpose of the GSD provided for adoption.

    To the best knowledge of GSDRGA, the GSD provided for adoption meets the GSDRGA
    standard requirements for GSD breed rescue. The GSD provided for adoption was rescued from a shelter facility that employs euthanasia as a form of population control, therefore the pedigree and/or registered standard of the GSD cannot be authenticated or guaranteed. Unless otherwise stated and provided, the breed registration of the GSD is therefore unknown and no registration papers are in the possession of GSDRGA.

  • Health Addendum

  • In accordance with the Adoption Agreement between         herein referred to as the Adopter and German Shepherd Dog Rescue Group of Georgia,
    Inc. (GSDRGA) concerning the adoption of     , herein referred to as GSD, GSDRGA will disclosure of any and all special health issues or
    concerns known about the GSD herein this addendum.

  • I agree that I have been fully informed of the health issues and concerns of the GSD and agree that I will abide by any special instructions given herein to accommodate the care and treatment of the GSD. I understand that sterilization/spay/neuter means that the GSD will be unable to breed or reproduce puppies.
    I am confirming that copies of all known veterinary records have been provided to me.
    I am aware that it can take up to six months after being bitten by infected mosquitoes before a dog will test positive for Heartworms. We recommend testing your dog for Heartworms 6 months after the most recent test. GSDRGA does not assume  responsibility for Heartworm treatment for a dog that tests positive for Heartworms after the dog is adopted.
