I agree that it is my responsibility to provide verification of enrollment for fall semester no later than 7-15-2025. This information will include my student ID number. See below for a list of what you may use as your verification.I agree to provide MGCF with a recent photo of myself to be used for Foundation publications, including our website, www.mgcf.org, and Facebook page.I agree to acknowledge my award with a thank you note/letter that includes information about myself and how this award will help me.I understand that the scholarship award will be mailed directly to my educational institution when scholarship checks are processed beginning mid-July. The Foundation will instruct the institution to divide the scholarship between fall and winter semesters. (Note - any awards $500 or less will be applied to fall semester only).I understand that it is my responsibility to notify MGCF of any changes in my address, phone number, and school transfers including withdrawing from classes.I understand that this scholarship may be revoked if I receive full funding from my educational institution or other sources. In no case may this award be used to reimburse another benefactor.
